SLA Claim Form
To submit your claim you will need to:
- Complete the Customer and Circuit info sections
- Select
from the drop down list the type of claim you are
making
- Complete the appropriate fields pertaining
to that type of claim
- Submit your claim between
the 1st and 15th days of the month following the
resolution of the incident
