SLA Claim Form

To submit your claim you will need to:

  • Select from the drop down list the type of claim you are making
  • Complete the Customer and Circuit info sections
  • Complete the fields pertaining to that type of claim
  • Submit your claim between the 1st and 15th days of the month following the resolution of the incident


Note: a separate claim must be made for each type of SLA violation. Select the SLA type, below, and then enter supporting SLA claim information on the appropriate tab for the selected SLA type
